This What’s Working Replication Guide shows how to form a Gatekeeper Program, which provides outreach and training to the business community and other community groups to help identify at-risk seniors and adults with disabilities, linking them to critical social and health services.

This program is one of 13 honored by The Aging Network’s Volunteer Collaborative for providing, in response to the 2012 Call for What’s Working, exceptional tips on how to build a strong volunteer program.
